Extremely Complicated Special Issuances - e.g. - Type 1 Diabetes - $150

NEW FAA approved Color Test (if Required) - Waggoner CCVT - $50

Reports Due (Special Issuance / CACI):
    Special Delivery  -->  $ 50 (i.e. FEDEX if needed)
     Electronic (FAA Upload) Delivery           -->  $ 25 to $50
CACI (e.g. - thyroid, blood pressure, colitis, asthma, etc  --> $20

Consultation - Email me First to Discuss Issue and Fee ($155 to $500)
Make phone call on your behalf  --> $55 per 10 minutes

No-Show or  Canceling Less than 24 hours -> $150 to $200 (my discretion) - You will be Invoiced
(Future appts will be cancelled until No-Show/Late Cancel fee paid)

NEW to Me - I might require PrePayment of $170 and at the visit if you need an EKG or have a SI/ CACI - that will be additional, and this is NON-Refundable - this is done to guarantee you Show to your Appt.
